A counseling case study, with identifying information carefully removed, showing your counseling theoretical perspective, biopsychosocial assessment, a collaboratively constructed treatment plan, course of treatment, and discharge summary.
Client – diagnosed with Severe Meth Use, and anxiety problem. Client did completed the 120 days program. He was discharged to a Sober Living Home that has an outpatient program. Client was able to find employment as well.
Presentations within the agency setting or at conferences or professional meetings.
Personal bibliography of books and articles read during your program.
APA style is not required, but solid academic writing is expected.
