The clinical issue that I chose is regarding adults with DM, glycemic control, and exercise. My PICO(T) question: In adult patients with DM (P), does regular exercise (I) compared to no exercise (C) lead to better glycemic control (O) within a 6-month time-frame (T)? In search for this topic, I first went to the Walden Library and performed a subject search of Nursing. Next, I searched “diabetes” which yielded 2.1 million results. I added the Boolean operator AND “glycemic control or hgbA1c or blood sugar or blood glucose”, this search yielded 350,000 results. I then changed the date range to 2018-2023 and checked the peer reviewed scholarly journals only box which narrowed it down to 113,000. Finally, I included AND “exercise or physical activity or fitness”, resulting in 10,000 finds.

My next search I went to the Walden Library and searched by subject of Nursing, then I chose Nursing Databases and there I selected CINAHL Plus with full text. I set up filters of publication date: 2018-2023, selected for age: All Adults, and chose Evidence-based Practice. In the search box I went ahead and used the same search words, but this time I used them all in the initial search which gave me 68 results. I added “no exercise or sedentary or inactivity” to this search and it narrowed down to 6 articles.

Strategies for conducting a rigorous database search to find relevant peer-reviewed articles would include using comprehensive databases like CINAHL, as it is one that is known to cover a wide range of medical literature (Hopia & Heikkilä, 2019). The use of Boolean Operators, filters, and limitations are another way to increase the rigor and effectiveness of a database search (Library of Congress, n.d.). Identify relevant keywords and synonyms for each component of your PICO(T) question and use those when searching the database (Stillwell et al., 2010). Moreover, after identifying relevant articles, check their reference lists for additional sources that may not have appeared in your initial search. Lastly, set up search alerts in your chosen databases to receive notifications of new research articles matching your PICO(T) question.
