Medications are: Atrovent inh, 2 puffs daily, albuterol nebulizer 2.5mg in h every 2 hours as needed, morphine extended release 30mg po twice daily, msir 20mg/ml 0.25ml sl every 2 hours as needed, dexamethasone 4mg po daily, 02 at 2l as needed, miralax 1 tbsp po daily, acetaminophen 650mg po every hours as needed, lorazepam 0.5mg po/sl every 4 hours as needed, furosemide 40mg po daily.

Assessment: A& O x3, L/s diminished throughout, rales in bilateral lower lobes, productive cough with yellow/brown sputum production large amount, grips are equal and weak, in bed 20 out of 24 hours, only eating bites, no taking liquids but is able to swallow, b/s are + x4, last bm three days ago, c/o fatigue, 2+ pitting edema bilaterally. Would like to see chaplain for spiritual needs. Experiencing nightmares once weekly.

STEP 2: CREATE A PLAN OF CARE FOR LUCY

ASSESS: 1) Identify assessments that you will complete for Julia using your textbook or other resources.(they must be cited in your references)

DIAGNOSE/PLAN: 2) Prioritize at lead three nursing diagnoses including the caregiving situation.

ACT or INTERVENE: 3) Develop nursing actions for the nursing diagnoses you have identified that are person focused for Julia.

EVALUATE: 4) Create anticipated outcome or evaluation statements for the plan.

STEP 3:

Once you have developed the plan of care, create a concept map/illustration/document of the care plan. This should be a simple, and easy to understand plan that you would share with Julia\’s family/caregiver.
