Health promotion activities that can be incorporated into the patient’s care plan include a comprehensive cardiovascular assessment, cancer screening for colorectal and prostate cancer, and the updating of their vaccinations as recommended by the Centers for Disease Control and Prevention (CDC) (Chiu et al., 2020). There should also be an assessment of the risk factors for diabetes and prevention strategies including proper dieting and weight management strategies, assessment of the patient’s mental health status and emotional well-being, bone health, and discussions on reducing the consumption or cessation of tobacco and alcohol use (Chiu et al., 2020). Lastly, the patient should be encouraged to engage in regular physical activity, maintain a balanced diet, screen for age-related chronic conditions, and set individualized health goals (Chiu et al., 2020).
How will you approach these recommendations with the patient?
The most effective approach would be carrying out a comprehensive assessment of the patient’s medical history, current health status, and lifestyle then engaging the patient in a conversation about age-related risk factors that they may have, their health concerns, and goals (Ferreira et al., 2022). Emphasis should also be placed on the importance of maintaining healthy habits to reduce the risk of cardiovascular diseases, diabetes, and other chronic health conditions. The patient should be advised on the benefits of the vaccinations recommended by the CDC, the risks and benefits of cancer screening, and the bone-related risks that come with aging. They should also be provided with the necessary resources and referred to the relevant healthcare providers to address any health concerns they may have (Ferreira et al., 2022).
What specific actions are important to incorporate patient needs?
Active listening will ensure that the patient’s health concerns and goals are properly addressed and incorporated into their care plan (Ferreira et al., 2022). The patient should be provided with information about the benefits, risks, and alternatives to their treatment to allow them to make informed decisions involving their care plan (Ferreira et al., 2022). Relevant materials should be provided to the patient to help clarify any questions or concerns that they may have.
What resources are available and provide evidence to support the suggested screening?
Available evidence includes the American Heart Association which provides guidelines on cardiovascular risk assessment, the American Cancer Society on cancer screening, and the US Preventive Services Task Force on diabetes screening.
